To register for the FLAP DISSECTION COURSE please fill in online Registration Form.

Complete the on-line registration, and order all the services you require: Registration fee, social program.

      

Please pay attention to the correct and complete completion of the form and e-mail address.

 

Upon receipt of the registration form, a confirmation of registration and payment information will be sent within 3 working days to the participants.

After receiving e-mail confirmation of registration to the Course, payment for registration must be made within 7 days.

The Registration fee includes entry to the Flap Dissection Course, Course Admission, Course Materials, Certificate of Attendance, Refreshments, lunch on 15 September 2021, Welcome Cocktail on 14 September 2021. 

 

PARTICIPATION

Capacity of the session is 24 people (2 participants per 1 station). Due to this limit all payments should be performed after the confirmation of the registration by the organizing committee. Do not make the payment until you receive the e-mail confirmation of the acceptance to the course.

 

The social programs must be booked separately upon registration.

CANCELLATION AND REFUNDS

Cancellation of participation and services must be submitted in writing to the Course Secretariat by e-mail: info@bos-congress.cz. The cancellation will be confirmed to you in writing. Cancellations by phone cannot be accepted. The appropriate refunds will be made after the course.

Refunds

Cancellation in the period without cancellation fees - a one-off administrative fee of CZK 100 will be deducted from your payment. The final amount will be sent to your bank account.

 

Cancellation after the fee-free deadline – after the deduction of cancellation fees the remaining refund amount will be returned to the account from which your payment was made.
